"I have trouble forcing myself to continue follow-up care... I go back [to the hospital] for a check-up each year and every year I have different problems, not with my health, but with insurance... I went in for my check-up July 1, 2004 and I am still paying the hospital off...
I tried to get approved through the hospital for a payment plan for the money I owed for my 2004 check-up at the time, but while management was reviewing the payment plan they offered to me, they dropped me in collections... I cannot afford to pay very much each month and it's only going to get worse when I have to leave my job [due to vision problems].
Assistance from The SAMFund will help me move forward. It will offer me a clean slate with medical bills that I will hopefully be able to stay on top of from now on."
